Application Forms

To apply for talking book service:

• Make sure that you meet the eligibility criteria.
• Print out the appropriate application information and application form.
• Read the application information, or have a friend, family member or service provider read it to you.
• Fill out the application form, or have a friend, family member or service provider fill it out for you.
• Have the application form certified by one of the professionals listed in it.
• Mail the application form to the address below, or ask your local public library to deliver it to us.

Application forms may not be submitted online or by fax.
